Healthy Together Now - Chronic Disease Prevention Initiative

The Healthy Together Now - Chronic Disease Prevention Initiative is designed to help communities address smoking, physical inactivity, and unhealthy eating, which are the three major risk factors that lead to chronic disease. The Healthy Together Now vision is to improve the health of Manitobans through local partnerships, citizen engagement, and community development. The mission of Healthy Together Now is to reduce the number of people in Manitoba who become ill or die from chronic diseases.

The goals of Healthy Together Now are to:

  • Provide a community-led approach to prevention of the major chronic diseases in Manitoba;
  • Implement a sustainable initiative that emphasizes partnerships;
  • Integrate Healthy Together Now into existing programs to make them even more effective; and
  • Enhance the ability of communities to address inequalities in health status.
